Acknowledgement & Authorization Forms

Parents and Guardians,

The acknowledgement and authorization of forms to finalize your student's registration is completed online through Skyward’s Family Access. By electronically verifying, signing and submitting the online forms, you and your student(s) agree to Judge Memorial's Code of Conduct, have the option to grant permission to the use of your student's images and other forms of media, acknowledge JPA volunteer service requirements for families, and authorize emergency contact to physicians listed in Skyward.

The Skyward Family Access to complete your student's registration is now OPEN for the 2018-2019 school year. Please follow the detailed steps below to complete this process.

Your student's schedule for the 2018-2019 school year will be released once all the steps have been completed and submitted. This process must be completed for each student you have enrolled at Judge Memorial in order for each schedule to be released. If submitted prior to July 9, 2018, your student’s schedule will be available on July 11. Otherwise, you may have at least a 48-hour processing period. 

Once the online schedule has been released, you may begin to order books for your student’s classes beginning July 10, 2018 via Judge Memorial book vendor, MBS Direct:

Please read and follow carefully all the directions below. If you do not have access to a computer, you may come to the main office to obtain a hard copy of the Acknowledgement and Authorization form. 

Note: Skyward may not allow for some updates (i.e. new employers) to be added by parents.  If there is any information that is not allowed and needs to be updated, please email Rita Scholl at to have this change made for you.

  1. Login to the primary parent or guardian account on Skyward:
    • If you are new to Judge, access codes and passwords were emailed to the email addresses listed on the application form.
    • If you have forgotten your password, visit: Click on: ‘Forgot My Password’.
  1. Click "registration and emergency contact" from the left side menu bar
  1. Select the student whose forms you wish to submit (NOTE: the steps included in this process must be completed for each student you have enrolled at Judge.)
  1. Review and update as necessary the information in section 1a. "Student Information"
  1. Click on the “Complete Step 1a only” button at the bottom of the form; if you make changes, you may encounter the following message in red; you may still proceed to the next step. "* If a Requested Change is denied by the district, Registration and Emergency Contact for this student will be marked as INCOMPLETE".
  1. Click the “Next Step” button located in the right side menu bar of the screen
  1. Repeat steps 4–6 for items 1b “Family Address”, 1c “Family Information”, 1d “Emergency Information”, 1e “Emergency Contacts”, and 1f “Health Information”
  1. Read carefully the below five agreements and authorizations. Type your name in each field under policies 1, 3, 4, and 5 to signify that you agree to each.

1) Code of Conduct and Online Responsible Use Policy: My Student and I agree to abide by the Judge Memorial Catholic High School Code of Conduct and Online Responsible Use Policy (accessible online, see pages 30 and 53 for details).

2) ** To Opt-Out of having your information posted in the online school directory, please use the “My Account” link in Skyward Family Access.

3) Media Authorization:  I hereby permit Judge Memorial Catholic High School to use, in whole or in part, photographs, videos, written extractions, and voice recordings of my student, in any medium, including but not limited to social media, for the purpose of promotion, marketing and other educational purposes of Judge Memorial.  Further, I permit the School to notify local newspapers of my child’s academic, athletic, and other special achievements. I agree that all rights of the above image(s)/sound(s) become the property of Judge Memorial. 

4) JPA Agreement: Each Judge Memorial family is expected to volunteer a minimum of 15 hours to the school during the school year There are many ways to fulfill this commitment. Opportunities are listed on the Judge Memorial website: Those families who do not complete the minimum of 15 hours will pay $150.00 in lieu of the volunteer hours

5) Emergency Authorization:  In case of Emergency, I authorize Judge Memorial Catholic High School to call the physician(s) I have listed on the family Access Skyward site at the time of registration or another if she/he cannot be reached immediately and follow her/his instructions. I further authorize the school to call an ambulance, paramedics, or the Fire Department and follow their instructions. I understand that the school does not assume any responsibility in the emergency procedures used and does not assume payment for any measures taken.

  1. Fill out the date at the top of the screen
  1. Type your name in each field under 1, 3, 4, 5 to signify you agree to each the policies of the school

  2. Review
  1. When done, click on the “Complete Step 2 Only” at the bottom of the form
  1. Click the “Next Step” button located in the right side menu bar of the screen
  1. Next is Step 3.“Complete Registration and Emergency Contact”: Review 

  2. Click "Submit Registration and Emergency Contact”

If after July 11, you are unable to access your student's schedule, be sure to check that the following are complete:

  • Active FACTS Payment Agreement for 2018-2019?
  • All outstanding fees paid with Finance Office?
