Physicals

PHYSICALS, PHYSICALS, PHYSICALS!!

Please note that Judge Memorial will be offering their yearly physicals on July 21st and 22nd. This year the physicals will take place at the University of Utah Orthopaedic Center located at 590 Wakara Way, SLC, UT, 84108, (801) 587-7005. The beginning time each day will be 4:30 and the cost will be $25.00. Please note that due to a change in school policy, ALL athletes will have to have a physical this year regardless of whether or not they have had one in the past. This is particularly important for fall athletes who will begin their sports in early August.

Mr. Del Porto will be on hand each day with all of the forms needed for participation. For fall athletes, once all of the paperwork is filled out, he will give you a white eligibility card, which will be the athlete’s permission to try out and participate in any given sport. Parents, particularly those of older athletes who may be able to drive, it would be very helpful to the process if you could be there as your signature is required on all of the forms.

Thank you for your attention to these important matters and please feel free to contact Mr. Del Porto if you have any questions. His direct line at Judge is 517-2172. His cell phone is 556-3341 and email is ddelporto@judgememorial.com
