Senior Parents: Baby Ads for Yearbook 2011

To reserve a space in the 2011 Yearbook for your senior's Baby Ad, please place your order by 10/29/10. A downloadable order form is attached.
There are two options available for ordering:
E-mail the yearbook staff at jmyearbook@gmail.com, and include your student's name, a typed message to your student (60 words max), your name and contact information, and a baby photo in .jpg or .tiff format.
Mail and/or deliver the attached form with the baby photo you have selected to Judge Memorial, Attn. Joan Jensen, 650 South 1100 East, Salt Lake City, UT 84102.
