The Judge Memorial Catholic High School-University Orthopaedic Center partnership, now in its second year, announces that yearly physicals will be held on two separate dates: Wednesday, July 29 from 5 p.m. to 7 p.m. and Monday, August 3 from 5 p.m. to 7 p.m. Cost per physical is $25.00. The University Orthopaedic Center is located at 590 Wakara Way, Salt Lake City, UT 84108.

All necessary forms will be available at the University Orthopaedic Center on the days of exams. Parents are encouraged to accompany their students to these exams as your signature, regardless of your child's age, is required on all eligibility forms. Thank you for your consideration.

Please note: All Judge Memorial athletes who intend to participate in any sports activity in the 2009-2010 academic year must have a physical this year, and every year, regardless of whether or not they have had one in the past. This is particularly important for fall athletes who will begin their sports season in early August.
