Physical Exams:
November 9, 2017 at 6:00PM at the UUOC for 2017-2018
 
Each year, students at Judge must meet several requirements to participate in athletics, one being a yearly physical exam. The University Orthopedic Center is providing an opportunity to get your athlete's physicals done for the winter/spring season! The physicals will be on November 9, 2017 at 6:00PM at the University Orthopedic Center. If you want your child to attend the physicals he/she must sign up with George Angelo in Room 318! There is a $25 fee which is due when the athlete signs up, so if no payment is provided they will not be added to the list.

When: November 9, 2017

Time: 6:00PM - If athlete arrives later than 20-30 minutes after 6PM, they will NOT be able to get a physical.

Payment: $25
 
When to sign up?: Every day up to November 9th; payment required to sign up!

To participate on an athletic team at Judge Memorial Catholic High School, students must complete required documentation from both UHSAA and Judge Memorial found online at registermyathlete.com.  Parents and student-athletes are reminded that participants are required to have a physical each academic year in order to be eligible for any athletic team at Judge. Physicals should be completed in May or June to allow students the full year of eligibility. A partnership between Judge Memorial and the University of Utah's Orthopaedic Center continues annually to provide an opportunity for all student-athletes to receive at minimal cost a physical for each school year.  Physicals signed and completed by doctors must be return to the Main Office at Judge Memorial.  Please remember all required documentation is required with a minimum of three days prior to tryouts for your particular sport.

Judge Memorial Catholic High School's new online athlete registration is available at registermyathlete.com. Please take a few minutes to complete a new Parent/Guardian Account which will allow you to register your student athlete. Instructions are below and in the attachments below. The Utah High School Athletics and Activities have moved to solely online-based registration with very few forms to return to the front office.  Please be sure to complete your registration form before participation in any tryouts.

  • Physicals (includes paperwork from the physician). This is the only item in the process that it turned in to the Main Office via hard copy. Once received, the school will input the information into the student-athlete's record.


Here are some basic steps to follow when registering your athlete online for the first time:

  1. Find Your School: Find your school by going to http://registermyathlete.com/schools/ selecting your state, and finding your school. Click on the school to continue to the next step. Schools are encouraged to make a direct link from their school's website.

  2. Create an account: Now begin creating your account by clicking the “Create An Account” button. After filling in the required information the system will automatically log you in and you will be required to accept the terms of use.

  3. Add a new athlete: The next step is to add an athlete. You can do so by click the “My Athletes” tab on the left-hand side of the page or by clicking “Add Athlete” underneath the “My Athletes” tab. This only needs to be done once during your athlete's entire career. The information entered here will carry over from year to year. This information includes your athlete's contact information and medical information.

  4. The athlete's profile: After you've created your athlete you will be brought to their Profile page. This page is a summary of their info and involvement.

  5. Register for a sport: Click “+ Register For A Sport” to begin registration, you will be asked to choose which sport your athlete is registering for.

  6. Your registration checklist: This page shows the status of your athlete's registration. You will be asked to complete several steps to complete registration including agreeing to documents, completing the physical, and answering the UHSAA's tryout checklist.

  7. Physicals: The UHSAA requires physical exams annually. Parents may download the appropriate physical form from RegisterMyAthlete, take it to a physician, and then upload it onto RegisterMyAthlete or turn it in to the school in person.

  8. Complete registration: Your registration is complete once all items on the checklist have been completed.

  9. After registration: After registration is complete, you can login at any time to view the Status of your athlete and their participation on the team.

If you have additional student-athletes to register, you can repeat steps 3-8 for them under your same account.

During the following years, once your athlete has been added to your account, you only need to follow steps 5-8 to register for sports.

Student Transportation

Judge Memorial Catholic High School provides official transportation to and/or from practices, competitions, activities, etc., when feasible to do so (“Official Transportation”). When Official Transportation is provided, students involved or participating are required to travel by the school’s Official Transportation method.

A student may be released from the Official Transportation method to travel with his/her parent(s) so long as the parents first obtain the coach’s/moderator’s approval, and complete the form as provided in the attachment below. Approval must be obtained at least 24 hours prior to the trip. No student is ever to be released to travel by themselves or with anyone other than the primary parent on record. Coaches or moderators cannot release a student through phone contact with the parent from an event, nor release a student to another student for transportation.

In instances where Official Transportation is not provided for an event or activity, students and their families must arrange for transportation on their own. The school, its administrators, coaches and/or moderators, cannot coordinate, consult, nor provide, any input on personal transportation arrangements.
